Mechanism of Action
Enclomiphene acts on estrogen receptor signaling involved in the hypothalamus and pituitary gland. By reducing estrogen-mediated negative feedback in this pathway, it can stimulate the release of LH and FSH.
These hormones signal the testes to support testosterone production and sperm development. The response depends on the underlying cause of low testosterone and the function of the hypothalamus, pituitary gland, and testes.
Potential Clinical Applications
Endogenous testosterone production
Clinical studies have investigated enclomiphene for increasing naturally produced testosterone in men with secondary hypogonadism.
Fertility considerations
Unlike exogenous testosterone, which can suppress LH, FSH, and sperm production, enclomiphene may preserve gonadotropin signaling in appropriately selected men. However, improvements in hormone levels do not guarantee improved fertility or pregnancy outcomes.
Enclomiphene and Clomiphene
Clomiphene contains two isomers: enclomiphene and zuclomiphene. Enclomiphene is the trans-isomer and has distinct pharmacological properties from zuclomiphene.
Although the compounds are related, their pharmacokinetics and effects are not identical. Evidence for one formulation should not automatically be applied to another.
